Medication Management Services Medication mix-ups are a leading cause of nursing home placement. Help senior members of your family take the right medications at the right time with our reminders and medication nurse program.
Nurse Medication Management
A registered nurse can help keep medications organized (hourly fee)
•
Arrange for medications to be picked up or delivered
•
Keep in-home medications and medication list current with doctor's orders
•
Prepour medications into daily reminder boxes or other medication dispenser
•
Develop medication calendars and reminder systems
•
Review current medications for possible drug interactions and monitor for side effects
•
Dispose of old drugs to avoid medication errors
Medication Reminder Service
Three types of reminders can be added to an existing Link to Life Emergency Response system:
Auto Reminders:
Free with Link to Life Emergency Response
•
Set the emergency response unit to beep at a preset time
Response center operators can call up to three times a day to remind clients to take their medications
Medication Dispenser with Monitoring:
We can add a medication dispenser machine to your existing Link to Life personal emergency response unit (additional monthly fee)
•
You can pre-fill the dispenser (or have our nurse do it) with up to four doses of pills per day
•
The dispenser beeps and opens so the client can take her pills
•
If the pills are not taken out, the emergency response center will be notified and will call the identified emergency family member or other contact person
